Private Patient
- Arrive 15 minutes prior to your appointment to fill out paper work.
- A prescription/referral is only required for Extended Health Care (EHC).
- If a physician has referred you, we will take a copy of the prescription/referral and you will retain the original.
Motor Vehicle Collision (MVC) Claims
- Arrive 30 minutes prior to your appointment to fill out paper work.
- Required information:
- Name of your insurance company (looking after the physical injury)
- Name and phone number of insurance adjuster
- Claim number
- Date of injury
- Extended Health Care (EHC – if any) and limits (ie. dollar amount)
- Prescription/referral required
- AB1 form (Notice of Loss and Proof of Claim form) needs to be filled out and sent in to your insurance adjuster within 10 business days of the date of your accident.

Workers’ Compensation Board (WCB) Claims
- Arrive 15 minutes prior to your appointment to fill out paper work.

- Required information:
- Date of injury
- The practitioner/hospital that first rendered treatment on you and the date of that treatment
- Referred by
- Date of referral
- Employer’s name, phone number and address
Canadian Forces
- Arrive 15 minutes prior to your appointment to fill out paper work.
- A doctor’s prescription is required.
- Need to bring your Canadian Forces member health card.
Veterans Affairs
- Arrive 15 minutes prior to your appointment to fill out paper work.
- A doctor’s prescription is required.
- Need to bring your Veterans Affairs member health card.
RCMP
- Arrive 15 minutes prior to your appointment to fill out paper work.
- A doctor’s prescription is required.
- Need to bring your RCMP member health card
